Laboratory

Labs we do not currently provide in-house:

1. Intermountain Healthcare serves as our Reference Lab for testing not performed in-house.
   Their testing dictionary can be found online at
   testmenu.com/intermountain.

1. EMR – Send as Future Order in Cerner
2. Scrip/Paper – Send with Patient/Sample
3. Fax order – (208) 359-6443
4. Phone – (208) 359-6442 (Verbal orders require a written confirmation and provider signature)

Orders must have the following information:

1. Patient Name
2. Patient Date of Birth
3. Lab(s) for testing
4. Diagnosis
5. Printed Provider Name
6. Provider Signature

Samples must be labeled with the following:

1. Patient First and Last Name
2. Patient Date of Birth
3. Date and Time of Collection

Average Wait Time:

1. Common tests performed in-house will be done within 24 hours
2. Most tests done at IHC take 3-5 days

1. Some tests have special handling needs
