We Refer Out:

1. Pediatrics (under age 13)

Sleep Clinic:

1. Patients meet with a provider
2. Provider diagnoses and sends for testing
3. Provider follows up with patient
4. Patients can self-refer, depending on insurance

Sleep Lab:

1. Strictly testing, no provider oversight
2. Primary care provider orders test

How to refer to us:

1. EMR or fax. The lab does not accept phone calls.
2. Once we receive the referral, we call and schedule the patient.
3. If we have follow-up questions, we call the clinic. Otherwise, we do not.

What to expect after referring to us:

1. Sleep Clinic: After meeting with the patient, we fax notes of the visit to the referring provider.
2. We try to reach patients by call or text 4 times. If we cannot connect, we send a letter to the referring provider letting them know we were unable to reach the patient and will stop trying.
3. Referring orders for the sleep lab are kept for 1 year.
4. Prior authorizations are valid for 3-6 months if the patient still wants to be seen after referral.
5. Sleep Lab: Once we receive an order from the referring provider, we must authorize it, which can take up to a week on average. Once approved, the sleep lab calls and schedules the patient.
6. Sleep Lab: If a patient no-shows or cancels at the last minute 3 times, we send a letter to the referring provider that we are dropping that patient.
